RESPIRATORY INDICATORS AS PREDICTORS OF SURVIVAL COPD PATIENTS DURING EXERCISE

Abstract
Aim. To study the significance of the parameters of the respiratory system in patients with COPD, determined during a step load test, in a comprehensive assessment of survival. Materials and Methods. 67 patients of both sexes were examined. Patients underwent a comprehensive determination of the parameters of the respiratory system: clinical scaling before and after 6MWT, instrumental methods, including capnography and pulse oximetry before, during and after exercise. Survival analysis was carried out on the basis of dynamic observation of patients for 5 years. Results. Аll indicators were higher in the group of patients with COPD (p <0.05). During exercise patients noted: dyspnea, weakness in the legs, heartbeat more often than in the comparison group (p <0.01). Dyspnea, as a reason for stopping/slowing down the pace during the 6MWT period more often by the subjects from both groups (p <0.05). Also, Dyspnea indices increased during performance of 6MWT, and in the comparison group they were significantly higher than in the control group (p <0.01). When analyzing the PETCO2 trend graphs we found the "periodic breathing" (PD). In the group of patients with COPD, the signs of PD in the analysis of the PETCO2 trend were found significantly more often than in the control group (p <0.01). In patients with COPD revealed the prognostic value of a comprehensive assessment of complex patients parameters (BMI, BODE, mMRS, Borg, FEV1, Tiffeneau 's index, signs of PH, 6MWT, signs of PD and desaturation during 6MWT). Conclusions: Dynamic capnography and pulse oximetry make to detect signs of PD in patients with COPD during 6MWT, which can deepen the integral assessment of the parameters of the cardio-respiratory system in patients with COPD in order to determine exercise tolerance, as well as the effectiveness of treatment. A comprehensive assessment of the survival rate of patients with COPD revealed the prognostic value of the following patient parameters: BMI, BODE index, mMRS, Borg, FEV1, Tiffeneau 's index, signs of PH, 6MWT distance, signs of PD and desaturation during 6MWT (p = 0.003).
