TARGETED ACTIVE CASE FINDING FOR TUBERCULOSIS IN CHILDREN IN A LOW INCOME URBAN COMMUNITY

Abstract
Tuberculosis (Tb) in children still remains largely undiagnosed and under-reported. The best control strategy is debatable, with passive case finding (PCF), active case finding (ACF), household contact investigations (HCI), singly and in combinations having variable outcomes. The National Tuberculosis Control Programme focuses mainly on PCF and recently, intensified case finding, mainly in health care facilities with no appreciable increase in case detection. Evidences exist that ACF increases Tb case detection but this approach has not yet been validated nor accepted due to fears of cost-effectiveness. This study targets a Tb endemic community and actively searches for Tb in children living within that area. A descriptive cross-sectional study, targeting a low-income community reportedly having infected adults on anti-Tb treatment and children living among them. Eighty-seven children were seen and examined, with ages ranging from 4 months to 14 years (mean 4.933.209). The male: female ratio was 1:1.1. None (0%) had symptoms suggestive of Tb. Two (2.3%) had chest clinical signs without cough nor fever. Mantoux test results ranged from 0-20mm with a mean of 3.47 1.1 and results ˃10mm were found in 17.6% of the children. Chest X-rays were performed by 46.8%, among whom 22.1% had abnormal results highly suggestive of Tb, with statistical significance (χ2=100.644 and p=0.000). Active case finding targeted at endemic areas is useful and should be implemented along with passive case finding to control Tb. Any chosen algorithm should consider the environment and its prevailing circumstances.
