TRAITS OF ALLELIC POLYMORPHISM OF CYTOKINE GENES IN WOMEN WITH RHEUMATOID ARTHRITIS OF UZBEK NATIONALITY

Abstract
Purpose of research: to study the G/A polymorphism of the TNFα gene, IL-1β (T31C) and
C-174G of the IL6 gene in women with rheumatoid arthritis (RA) of Uzbek nationality. Material and methods.
Patients with RA underwent examination in the departments of cardiac rheumatology and SCAL arthrology of
the 1st TMA clinic. The study included 77 patients with RA, the average age of 54.72 ± 12.32 years. The
average duration of the disease was 8.63 ± 7.56 years; in 71% of patients, advanced stage of RA prevailed
(71%). 39 patients were seropositive for rheumatoid factor (RF), while the average concentration of RF in the
serum of all patients was 207.22 ± 201.01 U/ml. The degree of inflammation activity was determined by DAS28,
the average DAS28 index was 5.85 ± 0.84 points. The average HAQ index was 2.63 ± 0.71. Extraarticular
(systemic) manifestations (VP) were detected in 10.4% of patients. The material for studying the frequency of
occurrence of the single nucleotide substitution (SNP) of the G/A TNFα gene, IL-1β (T31C) and C-174G of the
IL6 gene were samples of genomic DNA obtained from peripheral blood leukocytes from 77 patients with RA
and 81 healthy individuals (donors) kit for RNA/DNA extraction from “AmpliPrim-RIBO-prep” clinical
material. To detect polymorphism of the type I collagen gene, a polymerase chain reaction was carried out with
a kit of reagents for determining the G/A polymorphism of the TNFα gene, IL-1β (T31C) and C-174G of the
IL6C-174G gene of the IL6 gene. Digital material was processed using the software package “OpenEpi 2009,
Version 2.3”. Results. In women of the Uzbek population, the homozygous G/G genotype of the TNFα gene
(83.9%), the homozygous T/T (45.7%) and the heterozygous T/C (43.2%) T31C genotypes of the IL1β gene, the
homozygous G/G genotype predominate (67.9%) IL6-174 (G→C). In patients with RA, there was a tendency to
an increase in the frequency of the heterozygous variant of the TNFα gene, a decrease in the T allele and T/T
variant, an increase in the C/C variant of the T31C genotypes of the IL1β gene, a decrease in the frequency of
the homozygous G/G variant, heterozygous G/C and C/C were more often detected homozygous variants of the
IL6-174 (G→C) gene. Moreover, the seronegative variant of RA was characterized by a relatively higher
frequency of the heterozygous TNFα genotype (OR increased 2.1 times), the homozygous C/C variant of the
T31C genotype of the IL1β gene (OR increased 3.3 times), and more pronounced shifts in IL6-174
polymorphism (G→C) gene. The highest diagnostic significance of the combination of the T31C genes of the
IL1β and IL6-174 (G→C) genes was proved in predicting the effectiveness of basic anti-inflammatory therapy
of RA in women of the Uzbek nationality. Conclusion: various combinations of alleles of the promoter regions
of proinflammatory cytokine genes are associated with clinical and laboratory indicators of inflammatory
activity in RA
